We are improving healthcare in Dartford, Gravesham and Swanley (DGS) with the DGS Community Investment Programme. Following Lord Darzi’s recent report on the NHS, Dartford and Gravesham NHS Trust is already making strides, working in partnership across our hospital, community, primary care and voluntary sector.
Our goal with DGS Community Investment Programme is simple: better, faster, and more accessible healthcare for everyone.
To do this, we’ll be investing in three main areas:
- Stronger neighbourhood teams so people can get the wrap-around care they need in their community.
- More care at home with Hospital at Home and Community Virtual Ward teams, offering NHS care at home, avoiding hospital stays while still receiving specialist treatment.
- Extended same day emergency care services at the hospital to avoid unnecessary admissions.
These changes mean more proactive care, quicker recovery, less travel, and care closer to home for you.
