Are you serving in the Armed Forces (Regular or Reserve), a Veteran or a member of their immediate family or a carer? If so, we would like to hear from you.
The Trust has signed the Armed Forces Covenant and is therefore committed to ensuring you do not face any disadvantage in accessing healthcare.
We also have a number of staff who are trained as Armed Forces Champions, who will be able to talk with you, if that would be helpful, to ensure you are aware of all of the support that is available.
By completing the form below, we can ensure that your Armed Forces status is noted on your patient record. This will enable staff to ensure that you receive the appropriate care for your needs.

Please note - the Armed Forces community comprises veterans, reservists, cadet force adult volunteers, service leavers, individuals who are wounded, injured, or sick (WIS) and the partners or children of regular serving personnel. (The Merchant Navy is not included as part of the Armed Forces Community unless the merchant ship that they were attached to was involved in military action)
