Adult Band 5 Scenario and Care Plan Exercise
You are a qualified nurse on duty with three other trained nurses and a health care assistant, plus one 2nd year student nurse and one 3rd year student nurse.
One trained nurse is on her lunch break, one is in the process of discharging a patient (as there is a patient in AMU waiting to come into the bed) and the other is doing an admission of a adult to the ward.
You receive a telephone call from A&E informing you of an admission of a baby with a chest infection which may be bronchiolitis:
Joe is a 65 year old man with chronic lung disease who has home oxygen which is normally delivered at 1L/min. He is a known COPD patient. The A&E nurse hands over that his observations are:
- Respirations: 26
- Pulse: 128
- Temperature: 38.9◦c
- Oxygen Saturations: 87% in 2L oxygen via nasal cannula
- Joe arrived in A&E complaining of chest pain
A copy of the ward handover sheet, a map of the ward and a list of staff allocation for the shift are below.
List of staff allocation for the shift
Band 6 (in charge of the ward)
Currently in discharge with a patient
No patients allocated – co-ordinator of the ward
Band 5 - Number 1
Currently on lunch break
Allocated cubicles 5 and 6 – these are high dependency cubicles that are opposite the nurse’s station
Band 5 - Number 2
Currently admitting a another patient
Allocated beds 18, 19, 20 and 21
Working with a 2nd year student nurse
Rooms 18, 21 and 22 are single cubicles. Room 19/20 is a double bedded bay.
These are situated at the far end of the ward.
Band 5 Number 3 - YOU
Allocated beds 9, 10 and 12
Beds 7-10 are a 4 bedded bay to the side of the nurse’s station.
Cubicles 11-12 are single cubicles around the corner from the nurse’s station.
Health Care Assistant
Allocated beds 14 and 16.
Beds 13-16 are a second 4 bedded bay next to the first bay.
3rd Year Student Nurse
Allocated beds 1 and 2.
Beds 1+2 are a double bedded bay, cubicles 3 and 4 are single cubicles next to the nurse’s station.
