Cardiac Rehabilitation is one of the secondary preventions in reducing the risk Cardiovascular disease. Research shows that people who participate in Cardia rehabilitation, will become more physically fit, return to work and resume their social activity sooner. It will also help gain people’s confidence again after experiencing a huge event and we will support you physiologically.
There are four phases of Cardiac Rehabilitation:
At Darent Valley Hospital we can offer you a choice of Rehabilitation options:
Phase I- Pre-discharge
You should receive a hospital pack regarding cardiac rehabilitation on the ward or from the heart centre. This will give you a background about cardiac rehabilitation and other accessible resources.
Phase II – Early Discharge Phase
Once you have been discharged, we will contact you either by telephone or letter, inviting you to Cardiac rehabilitation and giving you an option to par-take.
Phase III – Community centre Exercise and Education Programme

- Seven- week programme based at a community centre in Greenhithe, kent (10 mins from Darent Valley Hospital)
- Individual assessments.
- Supervised group exercise sessions.
- Health education advice and information including how the heart works, healthy eating, medicines and stress management.
- Maximum of 15 patients on each course
Phase III – Home Based Programme

- Initial appointment and assessment at Darent Valley Hospital.
- Easy to follow step by step 6-week home programme.
- Own personnel heart manual and relaxation CD.
- Regular contact by telephone from Cardiac Rehabilitation Sister.
Phase IV – Referral to a Community Based Programme

- Run by qualified Phase IV cardiac fitness instructors.
- Individual assessment.
- Independent or group exercise sessions.
- Various locations available.
